In accordance with the North Carolina General Statutes (G.S. 130A-152 (a)), all immunizations for students must be current before the start of the school year.  If a student's immunizations are not on file with the school within thirty calendar days after the first day of school, the student will not be allowed to remain in school or participate in sports until the required immunizations have been completed.

Click HERE to view the list of required immunizations by grade level. These grade level required immunizations are are summarized below.

• Kindergarten: Students must receive their 5 year old boosters to attend school.

• 7th Grade Students: Students must receive a Tdap and Meningococcal Conjugate vaccine to attend school.
• 12th Grade Students: Students must receive their second dose (booster) of Meningococcal Conjugate vaccine to attend school.

The following vaccines listed below are recommended, but not required for your child.

All prescription and over-the-counter medications must be kept in the Health Room and dispensed by the school nurse (See Self-Carried Medication for exception).

If your child will be needing medication during school hours, please have your health provider complete the Authorization for Medication Administration (found in SLCA app or Family Portal under Resource Documents). This would include any midday ADHD medications, allergy medications, or as needed medications for various conditions that you would want available to be administered at school. This form and the authorized medication in original prescription or manufacturer's packaging may be submitted to the school when classes resume. 

Students MUST provide an updated action plan every year, signed by their physician with instructions for how to manage the illness at school. Action plans are required for chronic illnesses including allergies, asthma, diabetes, seizures, or comparable chronic illnesses.

You may utilize the SouthLake Christian Action Plan templates listed below, or you may request one from your physician. Whichever form you choose, it MUST be signed by the student's physician and updated every school year.

  • Students in JK-5th grade should have a set of EpiPens provided to the school along with the form.
  • Students in grades 6-12th are encouraged to carry their own EpiPen and emergency supplies). They will need to complete a Self-Carry Medications Contract.
• SouthLake Asthma Action Plan must be completed by your physician and submitted to the school by the first day of classes.
• Diabetes Emergency Action Plan), epilepsy (Seizure Action Plan), etc. please contact me by email. We will need to discuss your child's individual needs and ensure the general plans attached meet your child's needs. 

Every athlete must upload a current physical and medical information into Dragonfly in order to be able to participate in our Athletic Program.
